Desk Exercises Using the Community Health Worker Coverage and Capacity (C3) Tool in 43 Countries to Estimate CHW Workload: Experiences from World Vision International

CHWs are the backbone of primary health care, yet their workloads are often poorly understood or overlooked. To address this, World Vision applied the Community Health Worker Coverage and Capacity (C3) Tool, developed by USAID, across 43 countries to estimate CHW workloads and ensure their assigned tasks are realistic within available working hours.
Through desk exercises, field offices identified CHW responsibilities, estimated time requirements, and modelled different workload scenarios. The results revealed three key situations: CHWs whose workloads were manageable; CHWs overworked due to system-level factors beyond World Vision’s control; and CHWs overburdened in part by World Vision’s own programme demands.
These findings allow World Vision and its partners to take corrective action – from adjusting CHW population coverage and redistributing tasks, to advocating with Ministries of Health for policy or staffing solutions. By systematically using the C3 Tool, World Vision helps ensure CHW workloads are fair, sustainable, and aligned with global guidance, supporting better health outcomes for communities worldwide.
